Vendor Registration Instructions at U.S. Bank Stadium

Registering in the Minnesota Sports Facilities Authority (MSFA) vendor registration portal is the best way to keep your business top of mind when U.S. Bank Stadium partners are searching for vendors for current or upcoming projects. When a project opportunity arises that matches your business' product or service, you may be contacted about participating in our procurement process.

All companies are invited to register in the MSFA’s vendor registration portal.

The MSFA recognizes the legislature’s mandate that the MSFA shall make every effort to employ women and members of minority communities in the operations, management, and capital improvements of the Stadium, as more completely described in Minn. Stat § 473J.12. Women and minority owned businesses registered with the MSFA's recognized list of Certifying Agencies are highly encouraged to register as a vendor. The MSFA defines “certification” as an ownership structure that has been reviewed and approved by a third-party certifying agency as a diverse business. Suppliers are responsible for acquiring and maintaining a valid certification.

The MSFA provides the vendor registration portal free of charge at the MSFA vendor website. If you do not have internet access, you may call the MSFA at 612-332-0386 and ask that we mail a form to you. If you are hearing impaired, deaf, or blind and would like assistance from the MSFA, dial 7-1-1 and tell the communications assistant which type of relay call you wish to make. Filing Fee:THERE IS NO FEE TO REGISTER AS A VENDOR.

How to Fill Out the Vendor Registration Form

1. Type all information in the provided boxes. If mailing the application, please print legibly in black ink.

2. If you need extra space to complete any item within this application, use the space provided in the "Comments" section.

3. Answer all questions fully and accurately. If a question does not apply to you, leave it blank. Questions marked with an asterisk (*) are required fields.

4. If you do not have enough time to complete your application in one sitting, you may click “Save Progress and Continue Later” to finish your business’ registration at a later time. On the following screen, you will receive an Update Code. When you are ready to complete your registration, enter that code in the box at the top of the registration page to complete your application.

Registration Field Descriptions

Part 1. General Information:

Legal Business Name	State the legal name of your business.
Doing Business As	If you use a different business name other than your Legal Business Name, list it here.
Business Keywords (separate multiple by ‘;’)	This provides you with the opportunity to give more specific examples of the products or services your business provides. (i.e., if your business provides flooring materials and installation services, you may want to include "carpeting; hardwood; tile…etc".
Corporate URL	Provide your business' web address.
Federal Tax Number	Provide your business' Federal Tax Identification (ID) Number.
Address	Provide the mailing address of your business.
Office Phone	Provide the main phone number of your business.
Fax	Provide your business' fax number.
Email Address	Provide your business' contact email address (i.e., info@business.com)
Business Start Date	List the date your business was created.
Number of Employees	List the number of current employees, including owners. This includes full-time and part-time employees.
Contractual Capacity	From the drop-down menu, select the highest monetary amount your business will be able to contract with the MSFA.
Type of Organization	From the drop-down menu, select your business' organization type (i.e., LLC, LLP, S Corporation…etc.).

Part 2. Point of Contact:

Name	List the first and last name of the person you would like us to contact regarding upcoming projects.
Email	List the email address of the person you would like us to contact regarding upcoming projects.
Phone	List the business phone number of the person you would like us to contact regarding upcoming projects.
Cell Phone	List the cell phone number of the person you would like us to contact regarding upcoming projects.
[Note]	You may add additional names as a point of contact by clicking the 'Add Contact' box.

Part 3. Owner Information:

Principal's Name	List the business owner’s first and last name.
Email Address	List the primary email address of the business owner.
Phone	List the business or cell phone number of the business owner.
Fax	List the fax number of the business owner.
Title	List the job title of the business owner, if other than "owner".
Percent Owner	List the percentage of the business the owner holds.
Firm Subsidiary to Another Firm	From the drop-down menu, select if your business is a subsidiary to another business/parent business. If yes, provide the name of the parent business in the supplied field.
Who Owns the Controlling Interest in Your Firm	From the drop-down menu, select whether the controlling interest is owned by a business principal, a parent business, or an ownership group.

Part 4. Business Type:

Firm Type	From the drop-down menu, select your firm type. If your business falls under more than one category, select the one you most identify with.
Certified Business	From the drop-down menu, select whether your business is certified with an accredited certifying agency or not.
Ethnicity	From the drop-down menu, select your ethnicity. If you or your business falls under more than one category, select the one you most identify with.
Certifications	Select any and all certifications your business holds.
Certifying Agency	Select any and all certifying agencies who have certified your business.

Part 5. Products and Services:

From the drop-down menu, select the categories that best describe your business’ products and services. You may select up to three.

Part 6. North American Industry Classification System (NAICS) Codes:

A NAICS Code is a classification within the North American Industry Classification System. The NAICS System was developed for use by Federal Statistical Agencies for the collection, analysis, and publication of statistical data related to the US Economy. NAICS is a Self-Assigned System; no one assigns you a NAICS Code. Therefore, each individual business selects the code that best depicts their primary business activity and then uses it when asked for their code. If your Business Activities include more than one Unique Line of Business, you may want to use more than one NAICS Code. You may select up to four NAICS codes.
